Landscape Horticulture Course Material Shipment Form
Please fill out and submit this form as soon as possible. An initial shipment of course materials for the Winter 2012 semester will begin two weeks prior to the start of class. Students residing in Canada or the USA, who fill out the form after the start of class can expect course materials to arrive within a couple of days of filling out the shipment form. International shipments may take longer depending on customs requirements.
The courier company requires a signature for delivery of packages, and only delivers Monday through Friday from 8:00 a.m. to 5:00 p.m. If you are not home during these hours, please give us an alternative address – such as a business address – where someone can sign for your materials. We also recommend that you do not use a Rural Route number when providing a shipping address. If, however, a Rural Route address is your only option, it is imperative that you provide the emergency (911) number, legal land description or street number of your residence, as well as the street name and telephone number, or the courier company may be unable to deliver your package.
Shipping by courier will begin on December 19, 2011. It will conclude at the end of the day December 21, 2011 and resume in January 2012 when the University returns from the holiday shut down.
The following courses do not have materials to be shipped. If your course is not on this list, proceed to filling out the shipment form below:

- Business Management
Note: Address information provided for shipping will not be used for returning assignments. To permanently change your address, please complete the Address Change form.
